Monitoring pregnancy outcomes for women exposed to drugs, especially for newly marketed drugs, is important as it provides data to inform future clinical decisions about whether or not to use a particular drug during pregnancy. Large automated health insurance databases are an excellent resource to answer questions about the effect of maternal drug exposures on infants, and offer the following advantages over more traditional pregnancy surveillance methods:

• The size of the source data, which allows fairly rare exposures to be related to outcomes among infants
• Automated capture of both drug exposure and pregnancy outcome information through insurance claims, which diminishes issues of selective reporting
• The ability to ascertain pregnancy outcomes following exposure to any prescription medication, ensuring the surveillance precedes knowledge of the outcome.
